Join Elevate Care Providers LLC as a Private Duty Nurse (RN/LPN) providing one-on-one skilled nursing care in the comfort of a patient’s home. This role offers consistent hours, a low-stress environment, and the opportunity to build meaningful relationships while delivering high-quality, patient-centered care.

As a Private Duty Nurse, you will support patients with medically complex needs while promoting safety, comfort, and independence in a home setting. You’ll work closely with the patient’s care team and family to ensure continuity of care and excellent outcomes.

Responsibilities

• Provide skilled one-on-one nursing care in the home setting
• Administer medications as ordered
• Perform G-button / tube feeding administration
• Provide nebulizer treatments and suctioning
• Monitor patient condition and report changes promptly
• Assist with bathing, dressing, hygiene, and other ADLs as needed
• Follow the physician’s plan of care and nursing orders
• Maintain accurate and timely nursing documentation
• Communicate effectively with families, care coordinators, and supervisors
• Ensure infection control and patient safety at all times
• Patient Care Needs
• Ventilator-dependent care
• Tracheostomy care and suctioning
• Respiratory monitoring and support
• Medication administration
• Urostomy care
• Bowel program
• Passive Range of Motion (PROM)
• Assistance with ADLs (bathing, feeding, positioning)

Qualifications

• Active Missouri RN or LPN license (required)
• Experience in private duty nursing, home care, or skilled nursing preferred
• Comfortable with G-tube care, suctioning, and medication administration
• Strong assessment and communication skills
• Compassionate, dependable, and patient-focused
• Ability to pass background screening and meet state requirements

• Flexible scheduling available
• Days, nights, and weekends may be available
